PharmaPartners is a leading ICT-services provider focussing on the healthcare sector. For more than thirty years PharmaPartners has been developing advanced information systems, designed to provide hospital pharmacies, general practitioners, medical specialists, health centres, as well as patients and healthcare consumers, accurate, reliable, and up-to-date (patient) details. Working closely with the end-users, PharmaPartners has been able to contribute extensively towards the quality and effectiveness of today's healthcare system.

Organisation

More than 3500 health professionals in approximately 265 multidisciplinary cooperation schemes are currently using the services of PharmaPartners. Together they provide care to more than 8 million people in the Netherlands.

Challenge

With MijnGezondheid.net, PharmaPartners wished to develop a patient portal, facilitating online communication between patient and care provider, which would result in the full time availability of pharmacists and general practitioners. Patients should be able to view (part of) their personal health data, enabling them to actively manage their own dossier in a simple, quick, and secure manner. MijnGezondheid.net should also contribute towards the chronic care model, assisting the patient with the management of their condition.

With the development of MijnGezondheid.net, PharmaPartners has met the government's and insurers' objective to encourage patients to take responsibility for their own health dossier. This requires patients to communicate more effectively with their GP and pharmacist, and have permanent access to their current personal health details.

Solution

For PharmaPartners, Componence developed a patient portal based on Oracle WebLogic Portal. This fully customised solution uses a secure closed network on the internet, similar to systems used by the Tax office. Patients log in with their DigiD, allowing immediate and complete access to their own health data.Once registered, patients are able to enter parts of the pharmacist's and GP's dossiers, enabling them to make an appointment, keep track of laboratory results, or seek advice through an E-consult. A repeat prescription order service is available twenty-four hours a day.

Any additions from the general practitioner or the pharmacist to the patient's medical or medication dossier, will be immediately visible to the patient. Personal updates from the patient themselves remain invisible to the care providers.

MijnGezondheid.net also delivers information on drugs and health conditions, provided by Health Base and the Dutch College of General Practitioners (NHG).

PharmaPartners has successfully implemented MijnGezondheid.net at a large number of locations and, continues to work with Componence, constantly enhancing the patient portal with new functionalities, the latest plans involve integrating the portal with other care applications.
